- Have the physician sign and date the physician recommendation form. We have physicians we can recommend to help get you your Colorado Medical Marijuana Card!
- Photocopy a legible copy of an approved photo ID that establishes Colorado residency.
- Send in a non-refundable application fee of $15 (check or money order payable to CDPHE)
- Send all items above as certified mail to: Colorado Department of Public Health and Environment (CDPHE) in care of Colorado Medical Marijuana Registry
